Medication Requests - FOR EXISTING PATIENTS
Practice standards require all patients to continue to have regular follow up appointments for medication monitoring and refills. The appropriate time frame between appointments is part of your treatment plan discussed with your medication provider.
If you need a medication transferred to a new pharmacy, unless it is a controlled substance, the new pharmacy is typically able to transfer the prescription without your provider resending the prescription.
If your pharmacy has not kept the refill sent at your last appointment on file and needs it resent, please fill out the form below and list this as the reason.
Providers will typically provide refills from appointment to appointment in order to account for medication changes that may be made at your appointment. If refills are needed in between appointments due to missed appointments or postponing your appointment, it is at the discretion of your provider and there may be a fee of $20. Any medication request needs to allow 1 to 2 business days to be processed. It is your responsibility to attend your scheduled appointments and monitor your medications and not wait until you are on your last day of medications.
Patients on a controlled substance, such as stimulants (i.e.e Adderall, Ritalin), benzodiazepines (Ativan, Xanax), and hypnotics (Ambien), and other potentially addictive medications, need close monitoring with frequent follow-up. These prescriptions will continue to be refilled at your regularly scheduled appointment and are not typically provided without an appointment in order to comply with State and Professional regulations for usage of these medications.

Medication requests will be responded to by the provider sending in requested medication, if appropriate, in 1 to 2 business days. If necessary, the staff will contact you with further questions, so make sure to provide an up to date phone number for yourself.
